Recent prosecutions include a Work at Height case, a £75K fine for A-Plant, a £48k fine issued following a lifting operation incident and a prosecution involving a chemical spill.  Recent incidents include the terrible inflatable accident that resulted in two fatalities, an incident where a welder set fire to a £29m block of luxury flats, a case where a man was crushed to death by his lorry hydraulic equipment and a recent incident involving a migrant worker.
